How to complete the form

  • Please complete all sections of the form
  • Club Name must be as registered with your parent County FA.
  • County FA club ID (affiliation) numbers must be supplied to us before you play any sanctioned football.
  • First Choice and Second Choice Kit Colours must be different for Shirts, Shorts and Socks.
  • AGREEMENT TO BE SIGNED (League Rule 10) section must be signed by the Chairman and Secretary of the Club by inserting Signature and Address of the persons signing, as well as the full name of the Club.
  • The Club Secretary, Match/Fixture Secretary, Ground Details, First and Second Choice Colours will be published in the league directory which will be available online to the general public.
  • The completed form will be sent electronically to the League, Secretary & Chair.

Roles

  • Club Secretary details MUST be the same as registered with the parent County FA for your Club.
  • The Club Secretary is the main contact between your Club, the League, your parent County FA & other external football organisations.
  • The Match/Fixture Secretary (who can be the same person as the Club Secretary), will be the usual contact for the purposes of arranging fixtures between member clubs and teams.
